This commit does the following:

- sets all shell prompts in code-blocks to the root prompt
- uses shell-session code-block since the shell prompt was being
  treated as a comment
- links configure-aodh.rst in configure.rst (running tox was
  complaining that this file wasn't being linked anywhere)
- other minor cleanup

Installing source and dependencies

Install the source and dependencies for the deployment host.

  1. Clone the OSA repository into the /opt/openstack-ansible directory:

    # git clone -b TAG https://github.com/openstack/openstack-ansible.git /opt/openstack-ansible

    Replace ``TAG`` with the current stable release tag.

  2. Change to the /opt/openstack-ansible directory, and run the Ansible bootstrap script:

    # scripts/bootstrap-ansible.sh
