This commit does the following:

- sets all shell prompts in code-blocks to the root prompt
- uses shell-session code-block since the shell prompt was being
  treated as a comment
- links configure-aodh.rst in configure.rst (running tox was
  complaining that this file wasn't being linked anywhere)
- other minor cleanup

Configuring the operating system

Install additional software packages and configure NTP.

  1. Install additional software packages if not already installed during operating system installation:

    # apt-get install aptitude build-essential git ntp ntpdate \
  openssh-server python-dev sudo

  2. Configure NTP to synchronize with a suitable time source.
