Jesse Pretorius fb09d3528a Reference all current release documentation

This patch sets up the index to reference the documentation for each
release. This provides a clearer starting point for deployers who
need to follow documentation which meets their needs for either
a development-focused deployment, or a stable deployment.

OpenStack-Ansible's Liberty Series was first released with the 12.0.0 tag on 11 September 2015.

Kilo (current stable)

OpenStack-Ansible's Kilo series was first released with the 11.0.0 tag on 30 April 2015.
